The discharge planner or family members should keep these questions in mind when choosing an air ambulance company.

 

  1. Does  the air ambulance company have a medical director, accreditations, 135 certificate, professional liability, specially trained critical care nurses and medical staff?

  2. Is the aircraft chosen appropriate for patient's condition?

  3. What type of stretcher is on board?

  4. Is a full support system and necessary equipment on board, should the patient condition deteriorate?

  5. What is the response time on emergency and elective flights?

  6. How long has the company been in business and how many flights do they conduct each month?

  7. Will they arrange for ground ambulance arrangements?

 

Activating a Transport

 

  • Please be prepared to provide the following information
  • Name of contact, telephone number and location
  • Patient's name, age, weight, and diagnosis
  • Name of referring hospital, Physician & telephone numbers
  • Name of accepting hospital, Physician & telephone numbers
  • Please have the following forms prepared for our medical team
  • Physician order for transport, diagnosis & reason for transport.
  • Copy of hospital face sheet; include insurance & billing information
  • Lab work, X-rays, charts or ER records to submit to accepting facility
